Everyone will have their own pattern of symptoms.
We’re going to ask you some questions about physical problems you might be having since having COVID-19. You can skip questions if they don’t apply to you.
For example, if you often feel short of breath, or like you can't breathe properly. How much of a problem is breathlessness for you?
For example, a cough that won’t go away. How much of a problem is a cough for you?
This might be pain or tightness that appears suddenly, or comes and goes. How much of a problem is chest pain for you?
These are heartbeats that are more noticeable. Your heart may feel like it's fluttering, pounding, or beating irregularly. How much of a problem are heart palpitations for you?
Problems with memory and concentration
You might forget things easily or find it difficult to concentrate or multi-task. How much of a problem are memory and concentration for you?
Perhaps you have a headache that keeps coming back or is very bad. How much of a problem are headaches for you?
Perhaps you are having trouble sleeping. How much of a problem is lack of sleep for you?
